Active Members – New Benefit Cards 2022

A new law requires changes to your UEBT Active Member insurance card. Beginning January 1, 2022, you will notice the following new information added to your medical benefits card that will be specific to your plan of benefits:

  • Deductibles, including in-network and out-of-network deductible amounts, as applicable.
  • Maximum limits on out-of-pocket costs, including in-network and out-of-network out-of-pocket cost limits, as applicable.

New medical benefit cards will be available electronically beginning January 1, 2022, online in your account. Click the button below:

Follow these steps to access your electronic medical benefits card:
1. Click the yellow “Log Into Your Account” button above
2. On the menu to your left, click on the Correspondence tab
3. You can find a copy of the card under the Documents section

In the event you would like a new physical medical benefits card mailed to the address you have on record with the TFO, follow the following steps:
1. Click the blue “Order a Physical Card Here” button above
2. Complete the required information fields and select “Request Cards.”

For Members requiring Kaiser Medical Benefits Replacement Cards:
Call Kaiser Member Services at 1-800-464-4000 or go to Kp.org and request an ID card through their website. It will take 7-10 working days to receive your card.

UEBT Active Members will receive a new Elixir prescription benefit card in January, mailed to their address on record with the TFO.
