Tax Information: Your 2023 IRS Form 1095-B is now available online!
IMPORTANT HEALTH COVERAGE TAX DOCUMENTS!
You can view, download and print this form by going to the “Tax Documents” section under the Correspondence tab, located behind your secure login. Click below to get started:
To access your online form:
1. Click the “Log Into Your Account” button above
2. Log into your Participant Account
3. On the menu to your left, click on the Correspondence tab
4. You can find the form under the Tax Documents section
To request a paper copy be mailed to you:
1. Click the “Log Into Your Account” button above
2. Log into your Participant Account
3. On the menu to your left, click on the Correspondence tab
4. Under the Secure Messaging section, click the small envelope icon labeled “Create”
5. Select the topic Eligibility/Benefits
6. In the message itself type “Mail 2023 1095-B Form”
7. Click Submit. A secure message will be sent to the TFO
8. 1095-B form will be mailed to your address on file
9. Please allow 5-10 business days for mailing
(Please verify the Trust Fund Office has your correct mailing address by going to the “My Info” tab and reviewing the information in the “Address” box.)
